Becoming a New Patient
To register as a new patient at The Chapel Medical Centre you will need to complete a new patient registration form. Please click the link below and submit a completed form to join our practice.
You can also sign up for online services with us, allowing you to access your healthcare online.
Register for Online Access
Online services allow you to book or cancel appointments online, order repeat prescription and view parts of your medical record. The service is free to anyone registered with a GP at The Chapel Medical Centre and gives you another often more convenient way of contacting reception.
To sign up contact reception or speak to us. You will need proof of address and photo ID in order to sign up this can be done via a video call.
Change of Personal Details
It is important that we have up to date contact details in case we need to contact you.
Please let us know as soon as possible of any changes to your address, telephone number or name. Before submitting this form make sure your address still falls within the practice boundary.
